*TERMS AND CONDITIONS: Totals will be calculated by Gentek on May 30, 2025. ALIGN products returned between April 1, 2025 and April 30, 2025, and 30 days thereafter, will be deducted from total customer purchases. Totals will be calculated as follows: net value of purchases between April 1, 2025 and April 30, 2025, minus returns during the period starting on April 1, 2025 and ending on May 30, 2025. All orders must be invoiced within 30 days of ready date for coupon to be applied at invoicing. Credits will be applied in a “lump sum” amount to the customer’s account (including Terms accounts and COD accounts). Credits will be calculated on a cumulative basis and issued by June 2, 2025, based on the following criteria: 1.) $100 if at least 20 and not more than 39 boxes are invoiced, 2.) $300 if at least 40 and not more than 59 boxes are invoiced and 3.) $600 if at least 60 boxes are invoiced.
